Specimen Requirements

Submit 1 of the following specimens:

 

Preferred:

Whole blood

Container/Tube:  Lavender-top (EDTA) tube(s)

Volume:  5 mL of EDTA whole blood

Transport Temperature:  Ambient

Collection Instructions:  Avoid hemolysis. Patient should be asked to verbally state their name, whenever possible, at the time of collection to verify identity.

Note:  Label specimen with the following:

  1. Patient’s name (first and last)
  2. Date of birth
  3. Medical record number or social security number
  4. Date and time of collection
  5. Collector’s tech code (associate ID).  If the collector is a nurse, a qualified Blood Bank approved associate will add their tech code confirming specimen was collected according to requirements.
  6. BloodlocTM code (BloodlocTM code not required for prenatal outpatients.)

Container/Tube: Red-top tube(s)

Volume: 5 mL of blood

Transport Temperature: Ambient
